MM527 – Care Coordinator

This role will assume responsibility for coordinating the Company’s home care service in the Ellesmere Port Area.


  • Carry out the day-to-day co-ordination of the delivery of the team’s home care operations
  • Maintain awareness and knowledge of up to date operational issues and methods. In this capacity, to share the responsibility of ensuring home care operations comply with Health and Safety and emergency regulations and procedures as well as the Company’s operational policies and procedures
  • Ensure that clients whose condition has been reported as deteriorating by the Care Workers receive an immediate risk assessment and updated care plan where necessary.
  • Assist the Care Management to maintain records, produce reports and investigate complaints by service users and employees
  • Producing weekly staffing rotas
  • Apply and use appropriate systems for administrating and resourcing the delivery of the home care operation including records, computer reports, timesheets, supervision arrangements, shifts and rotas and work allocation arrangements
  • Effectively liaise with other departments of the company and with other employees of the Company and to key stakeholders such as commissioners and customers as well as the public at large.
  • Personally undertake out of office duties if and when necessary including providing emergency hands on care, undertaking on-call duties, attendance at social events when required and offering emergency telephone assistance to care staff as and when requested
  • Help ensure the quality of the team’s responses to the complaints or concerns of service users

Qualifications & Training

  • NVQ2 in relevant subject or working towards NVQ 3
  • Two year’s relevant previous experience in the private or public sector
  • Previous experience in a position involving the supervision of staff
  • Experience of using IT systems and applications
  • Proven ability to effectively supervise care staff
  • At ease with managing care staff with a warm, approachable and friendly manner
  • Able to effectively supervise and delegate to staff
  • Effective in dealing with colleagues
  • Checkable employment history
  • CRB and POVA clearance


  • Relevant professional qualification
  • Previous experience of coordinating care in either a private or public home care service
  • Previous experience in a supervisory capacity in the care sector
  • High level of organisational skills
  • Able to work flexible and extended hours if necessary
  • Owns own vehicle and is mobile

Hours of work

  • Standard 37.5 hours (9 am to 5 pm) and must be flexible
  • On call 3 days per week
